Widow and Orphan’s Pension

 Required Documents

• PD4 Application form.
• Original death certificate of death pensioner.
• Original married certificate.
• Original birth certificate of death pensioner.
• Original birth certificate of widow’s / widower’s
• Certified copy of the widow’s / widower’s national identity card.
• Certified copy of the widow’s / widower’s bank book.
• Widow and orphan’s membership card.
• If there is a change in name, an affidavit certified by Grama niladhari.
• If you have unemployed, unmarried children under the age of 26, their original birth certificate.
• Grama niladhari report on the current civil status of widow’s / widowers. (whether remarried and whether employed in government employment)
• Passport sized photos 02.
