Required Documents
- Completed application form with Rs. 25 stamp affixed
- Photocopy of National Identity Card
- Documents confirming ownership of the business premises
- Deed / License / Grant Letter / Lease Agreement
- If the document is not owned by the applicant, a letter stating the consent of the owner
- If there are no documents or licenses, a letter certified by the Grama Niladhari confirming permanent residence in the relevant place.
- Grama Niladhari report regarding the registration of the business name
- Receipt of money
Fee: Rs. රු.2,000.00
(Here, if the application is not submitted within one month of starting the business, a fine of Rs. 25.00 should be charged for each additional month)

- Business name registration certificate previously issued by this office.
- Grama Niladhari report regarding amendment of business name.
- Receipt of payment.
